Dr. Salbador offers individual counseling to anyone 18 years or older. The first visit is in-person and then you will have the choice to continue either with in-person visits or TeleHealth visits.

We accept payment for your visit from your insurance carrier with the balance due to be paid within 30 days. We require the deductible, copay, co-insurance payments and non-covered fees be paid at each visit. In the event of duplicate payment you will be reimbursed.

We accept the following insurance plans; however, please call your insurance company to verify your coverage and see if we are in-network. We do not accept Medicare, Medicaid, Trillium or Oregon Health Plan.

We do accept HSA/FSA for payments.

Appointment payments must be paid at time of service with checks, credit cards, or HSA/FSA cards. No cash payments accepted. 

If you have billing questions, please ask the office staff.

We charge a missed appointment fee if there is less than 48-hour reschedule notice. Please read the FAQ’s for more specifics.
